Contractors are required to fill out form 5512 and Installation Master Excel template found in the attachments section. If you would like to attend the site visit, please fill out the required forms and return to me NLT COB Thursday June 11, 2026.** AMENDMENT 0001 Pre-bid questions are due to me NLT 1100 PST on June 22, 2026.
